I hereby give the above named applicant permission to attend The Paul Riley Soccer School Indoor Mania. I verify to the best of my knowledge that the child is physically able to participate in the activities of the camp. I agree to allow my child to be treated, if necessary by a physician and/or trainer while attending. I understand that if this application is accepted there will be NO REFUND should I cancel. I, the undersigned waive and forever discharge the Paul Riley Soccer School, its staff, officer, agents, representatives, employees and successors from any rights and claims for damages to person, property, activities or while at the camp site.
